Instructions -- Either save this file on disk (on your web browser, choose "File," "Save As," then give it a file name and use the type "plain text"), or print it out. For each item, replace my notes with your information. (If you're hand-writing on a printed copy, just work around my comments.) Please do not leave anything blank (if you do, then I need to either track you down and ask you about it, or make it up myself), except perhaps "Vulnerabilities," which many monsters don't have.
