The LCAT registration form is in pdf format. The form is designed to be filled out using Adobe Acrobat. To download the form, right click on the form link and save to you computer. Once the form is stored onto your computer, double click on the form to open it up. Then fill out the form by entering the appropriate information. You can move through each of the fields by hitting the tab key. Once the form is filled out print the form, sign, and date the form and give it to a coach or the LCAT team registar.
