Creating Forms in Word

Go to View - Tool Bars - Forms

The  symbol is for a Text Form Field 
(You would use this for fill in the blank, add your name, etc.)
The next icon  is the Check Box Form Field 
The next icon  is the Drop-Down Field Form

The remaining icons allow you to draw in tables, insert tables and have shading in the forms fields.
The last icon  is Protect Form. This allows you to set the form in order to enable students can use the fields.



If you select the drop-down field form you must right click on the shaded area that is created and click on Properties.

Type in each drop-down item and click the Add >> button or the enter key.
The order of the items can be changed by clicking the up or down arrows.

Below is an example of a simple form. 

Note that it is not locked.



The form shown below is locked and is ready to be used by the students.

Font size and colors and highlights can be changed to suit your needs.
