Youth Teams receive Bollinger’s Insurance FREE with their
individual registration. With the
individual registration each youth
player/Coach receives their ASA Card that list
their date of birth, their insurance number with Bollinger’s along with the
deductible with is Zero. IF you have
insurance this will be a secondary insurance.

Remember it is your responsibility to be proactive when an
accident
occurs.
To get your claim form off line at Bollinger web site, from your
district commissioner or league. Fill it
out including all
information it request and then have your District Commissioner
sign off on it which is stating you do have insurance and are in good
standing with ASA. If you have to go to the
hospital take your card
and show it to them so they can get the
information from it. Don’t
wait 6 months to handle these matters. Don’t wait until the Doctor
is threatening to turn you into
collections. Immediately take care
of any claim you feel needs to be filed.

over Bollinger’s is because the parents or
coach did not follow up in
a timely manner with claim forms and
provides information and attaching bills.
With Adult Teams if you do not have personal insurance this is
a must for your team. Medical Cost has reached so high that$20
or $25 per players can be a life
saver. Who wants to get sued because
someone could not catch a ball, but blames you for their inability.
